Patient Forms

Whether you have seen a chiropractor in the past or are new to chiropractic, your team at Owens Chiropractic is dedicated to helping you improve your health and wellbeing.

Your Visit

Please arrive 5 -15 minutes early to your first appointment with the chiropractor, Dr. Adrienne Owens, so we can get information to bill your insurance and you can complete or turn in your new patient forms.

Your first appointment will be 50 minutes. Your chiropractor, Dr. Adrienne Owens, will complete your new patient evaluation then give you a chiropractic adjustment. After your first visit, your adjustment appointments will be 10 - 20 minutes.

If you are a new patient, you can save time before your first visit by downloading and bringing your completed forms with you.

1. New Patient form
   OR
   Motor Vehicle Collision Questionnaire (if you are getting chiropractic treatment due to a motor vehicle accident)
2. HIPPA form
3. Consent for Care form

Insurance

We are in-network for most medical insurance plans. If you want us to bill your insurance for your chiropractic treatment, please bring your ID to your first appointment.

1. Driver's license or state issued ID card
2. Insurance card
   OR Medicare card
   AND, if applicable, secondary insurance card

If you want to clarify your insurance benefits, whether we are in-network for your plan, your deductible, and co-pay, please call your insurance company or [call us] at 253-942-3300 a few days before your first appointment.

Payment

We accept cash, personal checks, and credit cards and welcome patients with and without medical insurance. Payment is due at the time of service. If we bill your insurance, your insurance co-pay is due when you come in to see the chiropractor.

If you don’t have insurance coverage for chiropractic treatment, ask us about our cash payment rates.
